Help us to evaluate your needs

In order to help us to evaluate your individual needs properly, all patients are asked to fill out and return a simple questionnaire.

The questionnaire is designed to be simple to understand, whilst ensuring that we are able to ascertain all of the information we need to provide you with the best advice possible.

There is a small charge for us to evaluate your needs, this will include your first months supply of your nutritional programme together with your treatment instructions which will be posted to you.

At the bottom of the questionnaire, there is a payment option, you can either pay online via Credit or Debit card via our secure payment page (please note credit and debit card payments will be processed in Sterling) or you may pay by cheque.

Which ever payment option you choose, there are instructions every step of the way to guide you through the process.

Prices depend upon your postal address, but currently are:

UK - £70.00

Europe (including Ireland) - £73.00

Rest of the World - £77.00
